New Patient Checklist

Getting started is simple. Follow the steps below to prepare for your first visit with MVMT + BALANCE. 

1. Schedule Your Initial Consultation

Contact us by phone or email to schedule your complimentary intake call. During this conversation, we'll learn more about your needs, answer any questions, and help you book your first appointment.

2. Provide Your Insurance Information or Select Cash Pay Option

Please share your current insurance information and let us know if you've recently changed insurance plans. We'll create your secure patient profile, where you'll be able to:

• Upload a copy of your insurance card

• Complete your new patient intake forms

• Review and sign consent forms electronically

3. Insurance Verification

Our team will verify your insurance benefits before your first appointment to help you understand your coverage and any expected out-of-pocket costs.

Please note that insurance verification is an estimate provided by your insurance company and is not a guarantee of payment. We recommend contacting your insurance carrier directly to confirm your specific benefits and coverage. 

Before Your Visit

To help maximize your insurance benefits, we encourage you to review your plan for any requirements, including:

• Co-pays due at the time of service

• Referrals from your Primary Care Provider (PCP), if required, may be faxed to 844-306-1358

• Annual therapy visit limits

• Prior authorization or pre-certification requirements

• Deductibles and co-insurance responsibilities

Payment Information

Our goal is to provide exceptional, affordable care while making the billing process as simple as possible. We accept several major insurance plans and will verify your benefits before treatment begins. Also, a transparent, fair cash pay rate will always be available for out of network members. 

Please note that MVMT + BALANCE may collect an estimated patient payment at the time of your visit based on your verified insurance benefits. This payment will be applied toward the cost of your treatment and, when applicable, will be credited toward your annual deductible and/or any coinsurance or copayment responsibilities.

Because insurance claims are processed after your visit, the amount collected is an estimate. Once your claim has been finalized by your insurance carrier, any remaining balance or credit will be reflected on your account.

MVMT + BALANCE is currently in network with the following insurance providers and programs:

• Medicare Part B

• Blue Cross Blue Shield (BCBS)

• American Specialty Health plans
